LKMGMA Sponsorship Opportunity

The Louisville Chapter of Kentucky Medical Group Management Associationdetermined at its January meeting that we would now have two sponsors at each meeting beginning in March 2010. In recent years, we have had a
list of businesses that want to sponsor a meeting but had to wait for a long period of time to do so. In an effort to offer a resolve to this situation, it was determined that by offering two sponsors per meeting,
this would allow members to sponsor a meeting in a more timely manner. Please contact Cheri McGuire for additional information at Cheri.McGuire@glms.org

2010 – 2011 MEMBERSHIP APPLICATION
(November 1, 2010 through October 31, 2011)

In order to apply for membership in the Louisville Chapter KMGMA, please pay the appropriate dues amount for your membership type: Active or Associate. Please use the definitions supplied with your payment buttons.

You can also pre-pay for all nine monhtly meeting (adds $189 to your dues). The 9 meetings included are: November, January, February, March, April, June, July, September and October. Just choose your meal type and add to your cart.

If you are joining mid-year and would like to pre-pay the remaining meeting fees, please pay the membership dues on this page. Then visit the Forms page, scroll to the bottom and select the Member Meeting Fee. You will be able to change the quantity to match the remaining number of meetings.

Active Membership Dues - $52.00

Active Member Description - One who works in a managerial or administrative capacity of a group of at least one physician engaged in the practice of medicine; or one who works for a consulting service whose principle practice is directed toward assisting and advising physicians on medical practice management issues. Price includes transaction fee.

Associate Membership Dues - $78.00

Associate Member Description - One who is employed by a company or corporation that does not function as a medical practice or consulting firm, but which does provide supportive services or products for medical practices. Price includes transaction fee.

OPTIONAL - Pay in Advance for all 9 Monthly Meetings

Members have the option of paying in advance for the 9 monthly meetings. The meeting fee is $21.00/meeting x 9 meetings = $189.00. Price includes tranaction fee. The 9 meetings included are: November, January, February, March, April, June, July, September and October.
